Go Ahead, Knock Yourself Out: Alcohol

A drink or two puts you to sleep faster. Then, a few hours later, it comes to collect.

It is a good night. Dinner ran long, the second bottle appeared without anyone quite deciding on it, and by the time your head reached the pillow you were fast asleep before you had finished the thought. This, you tell yourself as you drift off, is why alcohol helps. It helps you fall asleep easily.


And you are half right. That is the trap of the nightcap. The part you can feel, falling asleep fast, is absolutely real. The part you cannot feel, what happens over the next six hours, is where the drink quietly undoes the favor.


This series asks one question of everything people reach for at night, and it is a simple one: does it truly restore you, or does it only knock you out? We begin with alcohol, because it is the most common sleep aid in the world, and the one that fools the most people.



What the drink is actually doing

Alcohol is a sedative, and like every sedative it is good at the front door and careless with everything behind it. While alcohol’s sedative properties make falling asleep easy, the problem is that alcohol fragments the sleep stages your brain and body desperately need to become restored. You don’t remember dreaming, and that is simply because alcohol prevents your brain from entering REM sleep stages as it should normally. That is the signature. A scientific review that pooled twenty-seven studies found that REM sleep, the dreaming, restorative kind, is measurably disrupted by as little as two drinks and only worsens from there. The heavier the night, the deeper the front-loaded crash, and the sharper the rebound that follows it. Alcohol can also relax your throat muscles, and trigger snoring and worsening of sleep apnea. This villain is a poison to your liver and causes your heart rate to spike to help metabolize this poison. And while alcohol is a sedative that forces you into an early, unnatural sleep, the accumulation of its toxic byproduct, acetaldehyde, actively destroys sleep architecture. Alcohol also increases urine production, forcing more midnight trips to the bathroom that further break your sleep cycles. Finally, alcohol is very dehydrating, and higher doses can cause many features of a hangover, including thirst, fatigue, headaches, nausea, muscle aches, vertigo, and light/sound sensitivity. After too much alcohol you fall unconscious quickly, but your sleep quality suffers enormously, and next-day grogginess is almost guaranteed. 



The three-in-the-morning wake-up

You know the rebound even if you have never named it. You come up out of sleep somewhere around three, suddenly and completely, heart a little fast, and you cannot get back down. Maybe you blame the extra trip to the bathroom. But the deeper reason is that the sedative effect has worn off and your brain, released, swings the other way into shallow, fragmented sleep. The dreams that come in that stretch are often vivid and unrestful. That is the second half of the night trying, and failing, to give you the REM sleep the first half borrowed against.


So you were unconscious for eight hours and restored for maybe four of them, if you are lucky. In the morning the number on your sleep tracker is frightening and you cannot quite explain why, because you slept, did you not? You did. You just did not get the kind of sleep that repairs you.



The costs that outlast the hangover

The unrefreshed morning is the obvious cost. There are quieter ones.


As mentioned above, alcohol relaxes the muscles of the airway, which worsens snoring and can push ordinary breathing toward the stop-start pattern of sleep apnea; a 2018 systematic review and meta-analysis found that drinking measurably raises the risk of sleep-disordered breathing, fragmenting the night further for you and for whoever sleeps beside you. And then there is mood. Alcohol is a depressant in the pharmacological sense, and its relationship with low mood runs both directions: poor sleep is one of the most established risk factors for depression, with insomnia roughly doubling the long-term risk (Baglioni et al., 2011), and a nightcap that erodes your sleep quietly feeds that loop rather than easing it.


None of this is an argument for never having a glass of wine with someone you love. It is an argument against the specific belief that wine can help you sleep. It is not. It is helping you lose consciousness, which is a different thing, and it is charging you for the difference in the second half of the night.



If you want the sleep, not just the sedation

The practical fixes are unglamorous and they work. Give your body time to clear the alcohol before bed rather than using it as the thing that tips you over the edge; the later and heavier the drinking, the worse the rebound. Match drinks with water. And on the nights you want real rest, notice how you feel the next morning when you skip the nightcap entirely. That single comparison tends to be more persuasive than any study.


The larger point here is the one the whole series comes back to. The goal was never to be “knocked out”. It was to wake up restored, and the two are simply not the same.


The nightcap keeps its first promise and breaks the more important one. Falling asleep was never the hard part. Waking up rested is, and a drink cannot buy it for you.
